Position Summary

We are seeking a compassionate and dependable Certified Home Health Aide (CHHA) to provide exceptional care and support to clients in their homes or community-based settings. The ideal candidate will have experience in patient care, particularly with seniors and individuals with developmental or physical limitations. You will play a key role in promoting our clients’ well-being, safety, and independence every day.

Key Responibilites

  • Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and meal preparation.

  • Provide personal care such as catheter care, medication assistance, and vital sign monitoring.

  • Maintain a clean, safe, and comfortable home environment through light housekeeping and meal preparation.

  • Support clients with mobility using equipment such as Hoyer lifts or other adaptive devices.

  • Document care activities accurately in Electronic Medical Records (EMR) systems while adhering to HIPAA standards.

  • Observe and promptly report any changes in the client’s condition or behavior to supervising healthcare professionals.

  • Engage clients in meaningful activities that support mental, physical, and emotional well-being, including memory care for those with Alzheimer’s or dementia.

  • Collaborate effectively with nurses, care coordinators, and other team members to ensure a holistic approach to patient care.

Qualifications

  • Active CHHA certification in California (required).

  • CPR certification (required); first aid certification is a plus.

  • Prior experience in home health, senior care, hospice, or long-term care preferred.

  • Strong understanding of medical terminology and EMR documentation.

  • Compassionate, patient, and reliable, with the ability to provide dignified care in diverse situations.

  • Excellent communication skills with clients, families, and healthcare professionals.

  • Commitment to confidentiality and delivering consistent, high-quality patient care.

Other Requirements

  • Must be willing to complete and pass a Live Scan fingerprint background check as required by state regulations

  • Must provide proof of a negative TB test (within the last 12 months) or be willing to complete one prior to hire.

  • Current CPR and First Aid certification (or willingness to obtain prior to hire)

  • Proof of COVID-19 vaccination and booster (if applicable), or valid exemption documentation in accordance with state and company policy.
