Position Summary
We are seeking a compassionate and dependable Certified Home Health Aide (CHHA) to provide exceptional care and support to clients in their homes or community-based settings. The ideal candidate will have experience in patient care, particularly with seniors and individuals with developmental or physical limitations. You will play a key role in promoting our clients’ well-being, safety, and independence every day.
Key Responibilites
Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and meal preparation.
Provide personal care such as catheter care, medication assistance, and vital sign monitoring.
Maintain a clean, safe, and comfortable home environment through light housekeeping and meal preparation.
Support clients with mobility using equipment such as Hoyer lifts or other adaptive devices.
Document care activities accurately in Electronic Medical Records (EMR) systems while adhering to HIPAA standards.
Observe and promptly report any changes in the client’s condition or behavior to supervising healthcare professionals.
Engage clients in meaningful activities that support mental, physical, and emotional well-being, including memory care for those with Alzheimer’s or dementia.
Collaborate effectively with nurses, care coordinators, and other team members to ensure a holistic approach to patient care.
Qualifications
Active CHHA certification in California (required).
CPR certification (required); first aid certification is a plus.
Prior experience in home health, senior care, hospice, or long-term care preferred.
Strong understanding of medical terminology and EMR documentation.
Compassionate, patient, and reliable, with the ability to provide dignified care in diverse situations.
Excellent communication skills with clients, families, and healthcare professionals.
Commitment to confidentiality and delivering consistent, high-quality patient care.
Other Requirements
Must be willing to complete and pass a Live Scan fingerprint background check as required by state regulations
Must provide proof of a negative TB test (within the last 12 months) or be willing to complete one prior to hire.
Current CPR and First Aid certification (or willingness to obtain prior to hire)
Proof of COVID-19 vaccination and booster (if applicable), or valid exemption documentation in accordance with state and company policy.
