Licensed Vocational Nurse (LVN)

Position Summary
We are seeking a Licensed Vocational Nurse (LVN) to provide high-quality, skilled nursing care to patients in their homes and community-based environments. The LVN will collaborate closely with Registered Nurses, physicians, and interdisciplinary teams to develop and implement individualized care plans. This role requires compassion, clinical excellence, and a proactive approach to improving patient outcomes.
Key Responibilites
Provide direct nursing care in accordance with the patient’s care plan and under RN or physician supervision.
Administer medications, injections, wound care, catheter care, and other clinical treatments as ordered.
Perform regular patient assessments, monitor vital signs, and promptly report any changes in condition.
Communicate effectively with physicians, patients, families, and the healthcare team to ensure coordinated care.
Prepare and maintain accurate clinical notes and documentation in compliance with agency policies and HIPAA regulations.
Participate in case conferences and collaborate with interdisciplinary teams to update care plans.
Provide patient and family education on disease management, treatment, and health maintenance.
Support and supervise home health aides, ensuring care quality and adherence to standards.
Participate in on-call rotations and ensure proper arrangements for medical equipment and supplies.
Promote a safe environment and assist with patient mobility or transfers as needed.
Prepare clinical progress notes and communicate with the supervising RN or physician at least every 60 days.
Obtain and document new physician orders as patient needs change.
Coordinate care with community health resources and patient support systems.
Qualifications
Graduate of an accredited school of nursing.
Active California LVN license and CPR certification (required).
1–2 years of acute care or home health experience preferred.
Bachelor’s degree in nursing preferred, but not required.
Proven ability to supervise and instruct home health aides.
Excellent clinical judgment, observation, and documentation skills.
Strong communication, organizational, and problem-solving abilities.
Ability to lift, transfer, and assist patients safely; prolonged standing and walking required.
Must have a valid driver’s license, reliable insured vehicle, and meet state driving requirements.
Other Requirements
Must be willing to complete and pass a Live Scan fingerprint background check as required by state regulations
Must provide proof of a negative TB test (within the last 12 months) or be willing to complete one prior to hire.
Current CPR and First Aid certification (or willingness to obtain prior to hire)
Proof of COVID-19 vaccination and booster (if applicable), or valid exemption documentation in accordance with state and company policy.
