Licensed Vocational Nurse (LVN)

Position Summary


We are seeking a Licensed Vocational Nurse (LVN) to provide high-quality, skilled nursing care to patients in their homes and community-based environments. The LVN will collaborate closely with Registered Nurses, physicians, and interdisciplinary teams to develop and implement individualized care plans. This role requires compassion, clinical excellence, and a proactive approach to improving patient outcomes.

Key Responibilites


  • Provide direct nursing care in accordance with the patient’s care plan and under RN or physician supervision.

  • Administer medications, injections, wound care, catheter care, and other clinical treatments as ordered.

  • Perform regular patient assessments, monitor vital signs, and promptly report any changes in condition.

  • Communicate effectively with physicians, patients, families, and the healthcare team to ensure coordinated care.

  • Prepare and maintain accurate clinical notes and documentation in compliance with agency policies and HIPAA regulations.

  • Participate in case conferences and collaborate with interdisciplinary teams to update care plans.

  • Provide patient and family education on disease management, treatment, and health maintenance.

  • Support and supervise home health aides, ensuring care quality and adherence to standards.

  • Participate in on-call rotations and ensure proper arrangements for medical equipment and supplies.

  • Promote a safe environment and assist with patient mobility or transfers as needed.

  • Prepare clinical progress notes and communicate with the supervising RN or physician at least every 60 days.

  • Obtain and document new physician orders as patient needs change.

  • Coordinate care with community health resources and patient support systems.

Qualifications


  • Graduate of an accredited school of nursing.

  • Active California LVN license and CPR certification (required).

  • 1–2 years of acute care or home health experience preferred.

  • Bachelor’s degree in nursing preferred, but not required.

  • Proven ability to supervise and instruct home health aides.

  • Excellent clinical judgment, observation, and documentation skills.

  • Strong communication, organizational, and problem-solving abilities.

  • Ability to lift, transfer, and assist patients safely; prolonged standing and walking required.

  • Must have a valid driver’s license, reliable insured vehicle, and meet state driving requirements.

Other Requirements


  • Must be willing to complete and pass a Live Scan fingerprint background check as required by state regulations

  • Must provide proof of a negative TB test (within the last 12 months) or be willing to complete one prior to hire.

  • Current CPR and First Aid certification (or willingness to obtain prior to hire)

  • Proof of COVID-19 vaccination and booster (if applicable), or valid exemption documentation in accordance with state and company policy.
